GIVING OPPORTUNITIES

Patients, their families and friends often ask how they can express their appreciation to Home & Community Health Services, Inc. (H&CHS) for the exceptional care they received.  There are many ways of giving that can make it possible for H&CHS to offer the same compassionate care to others as you and/or your loved one received.
Cash or Personal Property
Gifts of cash or personal property are the simplest way to support H&CHS. You may transfer cash, securities, personal property, real estate, life insurance or other personal assets to the agency. These gifts are tax deductible.

Bequests
Many donors also make gifts through their wills.  A bequest is often the most realistic way for donors to provide a substantial contribution to H&CHS in the future. Many types of bequests are available to help you specify how your assets are distributed. You should consult with your estate planner or legal advisor to determine a plan of giving that is most meaningful to you.

Life Insurance
A donation of a life insurance policy is another way to give a substantial gift to H&CHS. To maximize your gift, you may contribute premium expenses on the policy, name H&CHS as owner and beneficiary, and receive an additional tax deduction for the premiums. If you wish to make a gift of life insurance, please contact us so that we may assist you.

Personalizing Your Gift
Contributions to H&CHS can be directed to a particular program, made in memory of a loved one or designated in honor of a caregiver. You may wish to consider the following options to personalize your gift:

The Honor Roll
Many patients wish to show appreciation to the nurses, rehabilitation therapists and home health aides who have provided them with compassionate, exceptional care. The Honor Roll pays tribute to the H&CHS staff members who have been recognized by patients and/or their families.

Memorial Gifts
A memorial gift in the name of a loved one can be a lasting tribute. Families may also specify in funeral notices that memorial contributions be made to H&CHS in addition to or in lieu of flowers.

Directed Gifts
Many patients and their families wish to direct gifts to a particular program that has personal meaning or is aligned with their charitable goals. Such programs include:

.  Community Outreach
.  Home Health Aide
.  Home Care

Directed gifts benefit specific programs through the recruitment and training of clinical staff and home health aides, ongoing education for current staff, and care for patients who would benefit from these specialized services, but are unable to pay.

 Matching Gifts
Many businesses support the charitable goals of their employees by matching gifts.  By participating in a matching-gift program offered by your employer or your spouse's, you can dramatically increase the impact of your contribution.  Matching gift forms are available from the employer.
