Evergreen boasts a 120-bed long-term-care unit, a 30-bed Subacute Rehabilitation Unit for patients in need of extensive therapy and care before returning home, and a 30-bed Memory Support Unit for residents with Alzheimer's Disease and similar forms of dementia.  Each of these units features its own dining and recreation area and is tastefully appointed with décor and furnishings that create a cheerful, homelike feel.

Long-term residents at Evergreen benefit from 24-hour nursing care.  An interdisciplinary team of staff members and physicians collaborate to meet each resident's unique needs. Evergreen also provides a comprehensive span of services, including rehabilitation, hospice care, intravenous (I.V.) therapies, wound care, respite care and more. Because Evergreen is located on the same beautiful campus as Johnson Memorial Hospital, residents also have immediate access to the Hospital's facilities and resources.

Subacute rehabilitation patients benefit from a full array of services, as well.  A cohesive interdisciplinary team strives to shape, coordinate and implement each step toward aggressive recovery, enabling patients to attain their highest level of independence and return to the comfort and security of their own homes.

Residents of the Memory Support Unit, which opened in November 2008, can continue to enjoy independence and freedom of movement thanks to the unit's safe, stimulating environment. The Memory Support Unit is staffed by experienced nurses and other professionals trained in the latest advances in Alzheimer’s care. Many special features and services have been implemented as a means to maximize stimulation, promote recollection, and overcome the spatial/visual challenges characteristically experienced by Alzheimer’s patients.

Evergreen is accredited by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO), a testimony of its high standards for quality.  The facility provides an effective and balanced response to the requirements of residents and patients, families and referring physicians.
